Rate data is derived from payer machine-readable files published under the Transparency in Coverage rule. Not reimbursement, billing, or legal advice. That line sits at the foot of every page on this site, and this page is what it means.

A figure here is what a payer published in its own file for a month. It is not a promise of what will be paid on a claim, it is not a fee schedule, and it is not guidance on how to bill. What gets paid on a real claim turns on the plan, the contract, the documentation and the review, none of which this site can see.

Code Hub pages summarise material that regulators and their contractors publish, which includes coverage determinations, classification guidance and documentation articles. A summary is a reading of a source, and the source governs. Where a page here and the governing local coverage determination or policy article disagree, the determination or the article is right and the page is wrong. Every page names the sources it was built from, so the governing text can be read directly.

Nothing on this site is medical or clinical advice, and nothing here decides whether a particular item is covered for a particular person. That decision belongs to the payer, under the plan that person is enrolled in.
