The joint DME MAC instruction is the operative one, and it is a billing instruction rather than a descriptor change: "Suppliers must discontinue use of A4351, A4352, and A4353 and utilize the new HCPCS codes A4295, A4296, and A4297 on claims for hydrophilic catheters with dates of service on or after January 1, 2026."
December 31, 2025 is therefore the last date of service on which a hydrophilic product could be billed under the legacy codes. A4351, A4352 and A4353 all remain active for non-hydrophilic products.
