Changelog

A dated record of what has changed in the codes this hub covers, and in the Medicare policy that governs them. One entry per change, each carrying the date it was announced, the date it takes effect where those differ, the codes it bears on, and the documents it was read from.

This is not yet a record of everything that has changed. The entries here were written because a code page needed them, so they cover the January 2026 urological code split, the April 2026 bowel management additions and the policy revisions around both. Older changes, and changes to codes with no page here yet, have not been written up.

An entry keeps its address. Where a later document overtakes an earlier one, the earlier entry says so and links the one that replaced it rather than disappearing, because a link to it may already have been shared.
