Codes
Nine HCPCS Level II codes for continence care, one page each. A code page states the long descriptor, the coverage determination and policy article that govern the code, the monthly quantity ceiling and the published Medicare allowable, and it cites the document each of those was read from.
A page marked as a summary in the last column has had its descriptor, its governing policy and its headline rules checked. Its coverage criteria, documentation requirements and fee detail have not yet been read against the governing determination in full. The page says so at the top as well, so nobody reaches the end of one believing it is finished when it is not.
Where a page and the governing determination disagree, the determination is right and the page is wrong. Every page names its sources so the governing text can be read directly.
